Visiting our Office

For the health and safety of our team and our patients, please note the following office protocols for all patients:

1. Paperless Patient Registration (myPateintVisit portal) - If this is your first visit, we require that you complete your online patient registration prior to your arrival. Please follow the instructions that were sent in an email.
2. Do You Need to Reschedule? - If you are experiencing any cold, flu or COVID like symptoms we respectfully ask that you call us to reschedule your appointment.
3. What to bring:
   1. All insurance cards
   2. All glasses and/or contacts
   3. Payment for any co-pay or co-insurance if applicable
4. Sanitization - All equipment is fully sanitized between every patient.
5. Mask - Not required for patients or staff.
